<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (1 to 5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>Major Duties: Reviews the Veteran's medication profile, allergies, adverse drug reaction history, laboratory data, diagnostic findings, and other pertinent clinical information to identify, prevent, and resolve actual or potential medication ­related problems. Performs clinical review of community-prescribed medications and associated documentation to support safe, effective, evidence-based, and cost-conscious medication use. Identifies high-risk medication issues, discrepancies, omissions, duplications, interactions, contraindications, and adherence concerns and takes appropriate action consistent with policy and professional judgment. Documents clinical interventions, recommendations, medication reconciliations, and pharmacist notes in the electronic health record and other approved systems. Coordinates pharmacy support for Veterans who receive medications or related treatment services through authorized community care. Supports continuity of medication therapy following community emergency care, hospital discharge, specialty referral, urgent care, and other episodes of non-VA treatment. Reviews, interprets, and verifies prescriptions and medication orders for accuracy, completeness, appropriateness, and compliance with applicable policy and professional standards. Ensures prescriptions and orders contain the information required for proper processing and patient safety. Applies formulary management principles when reviewing prescriptions, including evaluation of formulary status, therapeutic alternatives, criteria for ­use considerations, and non-formulary requirements. Supports prescription processing activities associated with community care medication requests in accordance with approved workflow and policy requirements. Clarifies questionable, incomplete, or clinically problematic prescriptions or medication orders with the prescriber or other appropriate source. Identifies and addresses medication safety concerns before dispensing, release, or further processing. Assists in ensuring proper documentation and processing of prescriptions involving non-VA providers, outside medication histories, and community ­originating prescriptions, consistent with current policy. Works collaboratively with pharmacy technicians and other support staff to ensure timely, accurate, and safe prescription workflow. Maintains awareness of pharmacy security, drug accountability, and controlled substance safeguards applicable to assigned duties. Participates in dispensing-related problem solving and workflow improvement related to community care and outpatient medication processes. Promotes safe medication use by reinforcing patient understanding of medication instructions and identifying barriers to adherence or continuity of therapy. Documents patient communication and education in the health record or other approved system when required. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
