<p>The St. Louis VA Healthcare System is seeking a Clinical Pharmacist that facilitates resolutions of Veteran concerns. They will be responsible for interpretation of provider medical orders, prescription order/entry/completion, patient compliance monitoring, patient education, provider drug information consultations, and patient medication therapy monitoring. NOTE: The 2-page Resume requirement does not apply to this occupational series. For more information, refer to Required Documents below.</p><h3>Major Duties</h3><p>Handles routine medication-related activities. Makes recommendations to clinical staff regarding medication therapy. Documents recommendations and interventions. Accurately fills or checks medication dispensing units, prescriptions, and aseptically prepared intravenous admixtures and other aseptically prepared pharmaceuticals. Fills or causes to be filled all physician orders and requests (inpatient and outpatient) in a timely manner. Track and find resolution in the Joint Patient Safety Reporting system (JPSR) pertaining to pharmacy service in a timely manner. Track and find resolution in Patient Advocate Tracking System (PATS) pertaining to pharmacy service in a timely manner. Documents all patient medication teaching sessions and consults in the patient electronic record. Provides drug information and education to medical staff, nursing staff, pharmacy staff, other health care personnel, patients and their representatives on medication-related topics as needed or assigned. This may include both informal and formal presentations. Assumes an active role in the overall pharmacy-training program by providing orientation, guidance and training to pharmacists, technicians, students, residents, etc., to ensure that pharmacy and medical center procedures and techniques are shared and learned to the necessary degree. Acts as a preceptor or mentor for pharmacy student and resident rotations. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
