ABOUT THIS ROLE
<p>The incumbent for this position functions at the highest level of clinical practice, working independently under their scope of practice to directly care for patients in internal medicine.</p><h3>Major Duties</h3><p>VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to the following: Designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments. Helping achieve positive patient centric outcomes through direct and indirect interactions with patient's providers, and interdisciplinary teams in assigned areas. Performing physical assessments. Ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Exhibiting initiative and resourcefulness in managing assigned workload. Providing high quality, patient centric care using evidence-based medicine to ensure safe, effective and efficient medication therapy. Interacting professionally with all customers Incorporating the VHA mission, vision and goals into own priorities and practice. Maintaining current, comprehensive knowledge of evidence-based practices, and actively pursues opportunities for continuous professional development.
