<p>The Clinical Care Coordinator/Nurse Geriatrics & Extended Care (GEC) Liaison evaluates clinical information, collaborates with interdisciplinary teams, and ensures that Veterans receive timely, appropriate, and well-coordinated care when referred outside VA. This role supports safe transitions, continuity of care, and the application of utilization management principles-promoting high-quality, cost-effective care.</p><h3>Major Duties</h3><p>Careers at Montana VA Health Care System Billings Chamber of Commerce The Nurse Liaison GEC Float plays a crucial role for the coordination and support of Veteran care across the continuum of services; ensuring that Veterans receive care in the least restrictive and most cost-effective environment, using established outpatient utilization management criteria, clinical practice guidelines, and VISN-wide clinical systems. Duties of the position are, but not limited to: Follows procedures per established policies and guidelines. Provides care in the least restrictive, most cost effective, through use of outpatient utilization management criteria, clinical practice guidelines and clinical pathways. Assist with significant collaboration between community and Veterans Integrated Systems Network (VISN) healthcare personnel and the transition of Veterans through the veterans VA healthcare system. Complete services within an organization that meets the requirements of national directives. Provide quality and performance measure analysis and report consult trends and systems issues through the utilization of the nursing process. Triage and care coordination of complex patients, work closely with Veterans to ensure smooth transition of care to community or VA facilities and care related referrals, improvement meetings, and service meetings. Collaborate with the interdisciplinary team to provide the delivery of patient care. Must be poised and articulate in communicating with all parties involved in the delivery of care, to include Veteran, family, medical staff, community partners, etc. Delegate elements of care to the appropriate individual based upon assessment needs, condition, and safety of patient. Provide peers with informal constructive feedback for improvement. Foster a safe and supportive environment conductive to professional development of health care professionals. Contribute professional nursing perspective with the interdisciplinary team. Partner with others for change and produce optimal outcomes. Support colleagues through knowledge sharing and to provide safe, quality nursing care. Share educational findings, experiences, and ideas with peers. Permanent Change of Station (PCS): Not authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
