ABOUT THIS ROLE
<p>The Nurse IV, Chief Nurse, Clinical Command Center (CCC), at San Francisco VA Health Care System (SFVAHCS) executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross services and/or discipline lines, and influence the organizational healthcare mission.</p><h3>Major Duties</h3><p>General Description of Assigned Duties: The Nurse IV, Chief Nurse, CCC, executes leadership and direct supervision to the Program Analyst and Nurse Manager for the Collaborative Care Case Managers that includes Utilization Management/ Utilization Review/Case Management/Discharge Planning/Transfer Coordinator/Bed Control duties, Bed Management System (BMS) Coordinator. Responsible for ensuring safe high-quality patient movement throughout SFVAHCS. Develops and utilizes strong collaborative relationships with physician counterparts. Assure competencies for all levels of patient flow staff working within HCS, assuring all have the correct documented competencies and UM/UR certifications are up to date. Supports the development of Evidence Based Practice Culture throughout the HCS. Requires knowledge of SFVAHCS policies and procedures, including Emergency Disaster Preparedness policies. Functions or Scope of Assigned Duties: Functions as an advisor, administrator, educator, consultant, and mentor who possesses a wide range of experience and leadership ability. Provides senior leadership and oversight for daily clinical and administrative operations within the Clinical Command Center under direction of the Chief of Staff office. Supports the integration and collaboration of nursing with other professional disciplines and functional areas in the mutual achievement of patient-centered and organizational goals. Collaborates with interdisciplinary team members, including other program and service leaders at the facility and network. Works effectively with all levels of staff, to include collaborative efforts with other health professionals, providing ongoing care management and coordination services. Relates effectively with customers, families, and the community as well as program and service leaders at the local and network levels. Participates in the development of CCC care delivery models and environmental design, ensuring evidence-based, accessible, affordable, and equitable patient care.
