ABOUT THIS ROLE
<p>The Caregiver Support Program (CSP) Program Manager (PM) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to the CSP, which encompasses the Program of Comprehensive Assistance for Family Caregivers (PCAFC) and the Program of General Caregiver Support Services (PGCSS) within the Veterans Health Administration (VHA).</p><h3>Major Duties</h3><p>Careers at Montana VA Health Care System Billings Chamber of Commerce The CSP PM is responsible and accountable for all elements of the nursing process when providing and/or supervising patient care. Duties of the position include, but are not limited to: Assess, plan, implement, and evaluate care based on age-specific components. Responsible for the coordination of care focused on patient and caregiver education, self-management, and customer satisfaction throughout the continuum of care. Oversee service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans regardless of period of service. Coordinate program implementation with multiple clinical services across all campus locations. Follow procedures per established policies and guidelines. Influence care outcomes by interdisciplinary team collaboration. Knowledge and participation in unit/program level quality improvement process and initiatives and customer service programs. Performance and leadership that impact the standard of care and safety of staff. Establish and maintain an ongoing education program for Veterans, caregivers, community agencies, students, and staff to facilitate understanding of clinical practice guidelines, mental health issues, readjustment issues and psychosocial issues facing caregivers. Develop and facilitate CSP focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for Veterans Affairs (VA) facility staff and the community. Develop and implement plans for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services (but who do not presently use VA for their health care needs). Conduct outreach to current PGCSS and PCAFC participants to inform them of available services. Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Subject matter expert (SME) to VA facility leadership and staff for all CSP related issues and serve as a consultant to other VA professionals and administrators on program specific activities. Serve as a liaison to the Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program. Collaborate with facility, VISN, and VACO to develop and implement solutions-participate in national, regional, and VISN operational calls; contribute to national leadership council committees; and implement cultural transformation initiatives. Monitor performance measures at the local, VISN, and VACO levels; provide responses to congressional inquiries; and briefings to providers, VA leadership, community organizations, and other internal and external stakeholders. Develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Oversee daily operations and manage all aspects of the CSP, to include clinical and business operations, accountable for program effectiveness, and modification of service patterns to comply with the delivery of standardized processes found within VHA Directive 1152(1).
