ABOUT THIS ROLE
Position Summary The Care Coordinator supports individuals by providing information, developing connections with community and behavioral health resources, and coordinating care services, including Medication-Assisted Treatment (MAT) and psychiatric care. The role also involves coordinating referrals and establishing aftercare and discharge plans.
