Advanced Practice Registered Nurse - Primary Care

<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Duties Performs assessments, engages in therapeutic problem-solving, counseling, consultation, and provides independent case management. Orders, interprets, and follows up on diagnostic tests Initiates consults to other disciplines Collaborates with physicians and other members of health care team Prescribes medications and other therapies Teaches patient, family, other staff and students Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems. BLS certification is required for this position and can be obtained upon hire Facilitation of homecare referrals and recommendations to support ongoing veteran needs Demonstrates professional communication and conduct Recognizes and supports suicide prevention initiatives Works collaboratively with all levels of health care professional and support staff to facilitate positive patient care outcomes. Compliance with various Isolation Precautions and Infection Control Guidelines Patient and caregiver education and support. n.
