ABOUT THIS ROLE
<p>The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) provides advanced evidence based care for patients in the General, Vascular and Thoracic (GVT) surgery clinics with additional support and Podiatry clinic as needed. The Nurse Practitioner evaluates, treats, and coordinates care across the perioperative continuum ensuring timely access high quality clinical outcomes and collaborative multidisciplinary care.</p><h3>Major Duties</h3><p>Duties include: Clinical Care Perform comprehensive history and physical examinations for preoperative postoperative and established surgical patients in General, Vascular and Thoracic (GVT)surgery formulate differential Diagnosis, order and interpret diagnostic test, and develop evidence based treatment plans. Manage perioperative care including wound care medication management post surgical follow up and coordination of specialty referrals. Provide same day access for urgent clinical concerns to support surgery surgical efficiency and reduce unnecessary emergency department utilization. Provide clinical backup for Podiatry as operational needs arise. Procedural responsibilities Perform approved clinical procedures within NP scope and privileges such as calling simple won't care staples slash suture removal dressing changes removal of drains and vascular access as well as other office based procedures including skin lesions, excisions, incisions and drainage, etc... Care Coordination and Collaboration Collaborate closely with attending surgeons residents nurses care coordinators and ancillary services to optimize patient flow. Communicate clinical findings and plans effectively with interdisciplinary teams. Participate in multidisciplinary surgical cases conferences when applicable. Patient education Educate patients and families regarding diagnosis treatment options perioperative expectations and long term management of a variety of surgical and non surgical conditions. Administrative and Quality responsibilities Document care and compliance with VA slash medical centers standards. Participates in quality improvement initiative population health reviews process redesign and clinic workflow optimization. Contribute to program development for the GVT surgery clinics. Track and manage consults, results and follow up to ensure timely care delivery. Professional practice expectations Maintain require licensure certification and privileging as a title 38 license independent provider. Participate and continue education performance evaluations and practice reviews. Provide coverage support for colleagues during absences when offered or rationally feasible.
