<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.</p><h3>Major Duties</h3><p>This position is being recruited as a multi-disciplined posting to be filled as either a Nurse Practitioner or Physician. This Advanced Practice Nurse position is located within the Primary Care Service line located at the New Bedford CBOC for the VA Providence Healthcare System. This position is full-time at 40 hours per week. The incumbent practices in Primary Care Service as a Advanced Practice Nurse (APN) to provide a full range of services emphasizing quality, timeliness, and the advancement of the VA mission. The APN provides advanced clinical practice, consultation, and management. The APN practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. Duties include, but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice; Develops and implements an individualized plan to achieve optimal population health and disease management; Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members; Orders and interprets diagnostic studies; Prescribes pharmacologic and non-pharmacologic interventions; Initiates referrals/consults to specialty services; Provides education to patients and their families; Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes; Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence; Other duties as assigned. If your school has changed names, or is no longer in existence, you must provide this information in your application.- If your school was accredited by ACEN or CCNE at the time of graduation, but is no longer listed on the websites, you must provide documentation of accreditation with your application packet.
