<p>The Advanced Practice Nurse (APN) Psychiatric Mental Health Nurse Practitioner (PMHNP) assigned as PCMH-I Program Manager serves as the administrative leader of a multidisciplinary team including nurses, psychologists, psychiatrists, doctoral level pharmacists, and Nurse Practitioners and also has primary responsibility for the operation and management of key clinical, training, and administrative programs within the Primary Care setting.</p><h3>Major Duties</h3><p>Duties for this position include, but are not limited to: Independently practices as a psychiatric mental health nurse practitioner providing a full range of psychiatric mental health services utilizing treatment guidelines in the delivery of high-quality care. Collaborates with patients, families, and health care practitioners to develop, implement, and evaluate care. Utilizes customer service principles, supports veteran self-determination, and serves as a resource for patients and staff in addressing ethical issues. Demonstrates critical thinking in assessing and managing complex situations. Independently practices as a psychiatric mental health nurse practitioner, utilizing evidenced-based practice guidelines in the delivery of high-quality care. Collaborates with patients, families, and the health care team to develop and implement treatment plans and evaluate health outcomes. Leads the delivery of care to assure continuity of care and peer accountability for practice, improving outcomes of panel/program. Provides leadership in the application of the nursing process to client care, organizational processes and/or systems, improving outcomes at the program or service level. Contributes to performance improvement initiatives by identifying, analyzing, developing, and implementing processes that improve care at the panel, program, or population level. Incorporates research, evidence-based practice, established professional, community, or regulatory standards, and clinical practice guidelines to validate or change clinical practice. Demonstrates positive, effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers. Works effectively as a leader in planning and facilitating change to improve outcomes. Collaborates with stakeholders to improve patient care, staff development, and performance measures. Participates in professional development in the areas of practice, education, administration, and knowledge acquisition. Educates colleagues and/or students and serves as a resource to interprofessional team members. Develops and assumes responsibility and accountability for evaluating progress toward professional career goals. Implements an educational plan to meet changing program or service needs for self and others. Maintains knowledge of current techniques, trends, and professional issues. Uses professional standards of care and practice to evaluate programs and/or service activities. Permanent Change of Station (PCS): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
