<p>The Care in the Community (CITC) services Advanced Practice Registered Nurse - Nurse Practitioner (APRN-NP) coordinates Non-VA care in the community to include GEC, inpatient, outpatient, and emergency services. The CITC NP operates under Delegation of Authority as assigned by the Chief of Staff.</p><h3>Major Duties</h3><p>Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements and individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. The APRN is a registered nurse who has completed the necessary educational preparations to engage in primary health care decision making; obtain advanced competencies and skills for assessing physical and psychosocial health/illness status of patients; promote health maintenance and protection against disease; manage the care of Non-VA care patients within the scope of preparation and accountability; and participate in quality management/improvement activities. The APRN will be in daily contact with other providers and health care staff with regards to Non-VA care related activities. The APRN utilizes advanced practice nursing skills and knowledge in a relatively independent manner in areas of patient care, education, research and consultation. The APRN possesses a high degree of knowledge, skill, and competence in evaluation and management of complex acute and chronic disease and is knowledgeable of Non-VA care guidelines. The APRN provides leadership in the application of the nursing process to client care, organizational processes, and/or systems to improve outcomes at the program, service, or organizational level. The APRN is adept at interpersonal skills, working closely with all members of the CITC interdisciplinary team and functions as a mid-level provider in the CITC. The APRN is competent in procedures and required activities consistent with practice guidelines and standards of care established for the specific practice area. The APRN has an individualized scope of practice and is responsible for working collaboratively with inpatient and outpatient medical teams. The incumbent serves as an agent of the COS regarding prescriptive authority. Prescriptive authority will be delineated within the APRN's individual scope of practice. The delivery of care provided by the APRN is in accordance with established standards of care, and medical center policies and procedures and in collaboration with the plan of other health are providers. Permanent Change of Station (PCS): Not Authorized.</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
