<p>THIS IS NOT A REMOTE POSITION AND WILL BE LOCATED AT THE MILWAUKEE VAMC. The GEC Home Telehealth RN Care Coordinator executes position responsibilities that demonstrate leadership and creative approaches to the management of complex patient care. Care will be provided in the least restrictive, most cost-effective manner. HT is a program that applies care and case management principles to coordinate care using informatics, disease management, protocols, and individualized nursing interventions.</p><h3>Major Duties</h3><p>THIS IS NOT A REMOTE POSITION AND WILL BE LOCATED AT THE MILWAUKEE VAMC. Major duties and responsibilities: Provide initial and ongoing comprehensive assessments to include a review of systems which establishes a comprehensive plan of care. Lead and organize delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice. Apply advanced clinical knowledge/judgment to lead staff involvement in planning, decision-making, and evaluating outcomes beyond the immediate practice setting. Provide communication and data exchange with community-based providers when there is co-managed care. Lead initiatives to improve the Veteran experience i.e., care coordination, timeliness to resolve issues, negative trends, or environment of care concerns, beyond the immediate practice setting. Lead staff to identify, analyze and address ethical issues that impact patients, caregivers and staff beyond the immediate practice setting. Demonstrate positive, effective communication skills, professional behaviors and leadership that promotes a commitment to excellence, improving nursing practice, and fostering collaboration among customers beyond the immediate practice setting. Provide interdisciplinary consultation and interventions such as with Home Based Primary Care (HBPC) and other non-institutional care programs and venues, Mental Health (MH), Social Work (SW), Pharmacy, and Nutrition. Lead with emotional and social intelligence. Demonstrates resilience in coping with challenging and stressful situations, to assist others in increasing their self-determination and competence beyond the immediate practice setting. Excel in making appropriate judgements. Demonstrates integrity in the workplace and facilitates formal and informal mentoring opportunities to develop current and future leaders beyond the immediate practice setting. Initiate and lead interdisciplinary teams that utilize critical thinking to improve nursing practice beyond the immediate practice setting. Communicate and facilitates with the interdisciplinary team regarding progress to goals; patterns or trends of data, symptoms or findings; concerning findings; need for provider assessment and/or interventions. Demonstrate expertise in evidence-based practice/research resulting in improved outcomes beyond the immediate practice setting. Demonstrate expertise in managing resources that contribute to efficient, cost-effective care and, sharing knowledge beyond the immediate practice setting. Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized</p><h3>Requirements</h3>U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
