ABOUT THIS ROLE
<p>The Community Living Center and Hospice Registered Nurse delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.</p><h3>Major Duties</h3><p>Community Living Center and Hospice Registered Nurse duties and responsibilities include, but are not limited to: Community Living Center (CLC) and Hospice Registered Nurse (RN) is responsible for and accountable for all elements of the nursing process providing competent, evidence-based care to residents with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. The CLC and Hospice Staff RN considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. They administer medications and procedures per established policies and guidelines, influences care outcomes by collaborating with members of the interdisciplinary team. The CLC and Hospice Nurse is an RN who will utilize the nursing process and evidence-based practice in order to develop individual plans of care for patients in the CLC and Hospice. They are responsible for patient advocacy and collaboration with other team members, promoting effective communication, and ethical practice The RN provides rehabilitative, restorative and supportive/bereavement nursing care to prevent deterioration, when possible, to achieve and maintain optimal levels of functioning, independence and comfort, maximizing the Veterans' ability to adapt and adjust to living as independently and safely as possible. The RN provides ongoing bereavement support, recognizing each Veteran's desire for autonomy and their right to dignified care throughout the end-of-life process.
