The oncology clinical pharmacy specialist (CPS) functions at the highest level of clinical practice and works independently based on a practice scope as authorized. Is a clinical oncology expert and will work closely with the Oncology Pharmacy Supervisor to maintain the program and to educate pharmacy employees.</p><h3>Major Duties</h3><p>The oncology clinical pharmacy specialist (CPS) functions at the highest level of clinical practice and works independently based on a practice scope as authorized by the Medical Center medical staff leadership. A CPS serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy, and serves as a consultant for intensive medication therapy management. Responsibilities include, but are not limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams; and ordering, assessing and responding to laboratory and other tests to help determine efficacy and toxicity of medication therapy. Will have knowledge of best oncology pharmacy practices endorsed by American Society of Hospital Pharmacists (ASHP), American Society of Clinical Oncology-Oncology Nursing Society (ASCO-ONS), VA performance measures, and Pharmacy Benefits Management (PBM) initiatives and integrate these standards into practice. Functions Provides direct patient care, independently designing, implementing, and monitoring therapeutic drug plans. Initiates orders for medications, medication changes, and laboratory tests within authorized scope of practice. Develops and maintains patient-specific pharmaceutical care plans for Oral Hematology/Oncology patients. Reviews all inpatient and outpatient medication orders, including oncology drugs, for appropriateness. Identifies, addresses, and resolves drug-related problems through coordinated processes. Documents recommendations and all interactions in the electronic health record as progress notes. Completes medication reconciliation and provides patients with updated medication lists. Participates in Adverse Drug Event Reporting and documents medication errors to enhance patient safety. Reviews and acts on Prior Authorization Drug Requests (PADRs), documenting approval or disapproval. Counsels patients and families on medication use, disease state management, and necessary lifestyle modifications. Provides specialized education and training to healthcare professionals on oncology care and chemotherapy. Delivers authoritative drug information and resolves drug therapy questions for healthcare providers. Regularly reviews current literature to stay informed of new drug treatments and pharmaceutical practices. Maintains self-directed education to support continual professional development as a clinical pharmacy specialist. Trains and mentors pharmacy residents and students as assigned. Assists in designing, conducting, and presenting medication use evaluations and research projects. Collaborates on investigational drug studies, including the compassionate use process and oncology drug administration.
